Accessing Tax Forms & Notices

• Tax Forms: Provide a breakdown of taxes for services and fees.
• Notices: Contain important updates and additional information about your statements.

To view Tax Forms & Notices:

1. Log in to Lighthouse Business Manager.
2. From the left navigation menu, go to Reporting > Tax Forms & Notices.
   
3. If you manage multiple locations, select the location(s) from the Location dropdown and select Confirm.
4. To change the year, update the Year dropdown.
5. Select the download icon next to the tax form or notice you want to download.
